Well,

The shoulder patch of the soldier carrying this item seems to be unreadable.

Then the weapon appears to be a grenade launcher (30 to 40 mm). There is what appears to be a carrying handle. But there aren't any visible sights. So, perhaps a prototype, as it certainly isn't the six-round revolver grenade launcher developed by South Africa and in use with the USMC.
